INDERAL® LA (propranolol HCl) Long-Acting Capsules belongs to a class of drugs called beta-blockers. These drugs lower blood pressure (hypertension) by blocking the effects of the hormone epinephrine, also known as adrenaline. Beta-blockers cause the heart to beat more slowly and with less force, thereby reducing blood pressure. Beta-blockers also help blood vessels open up to improve blood flow.

INDERAL® LA is a long-acting form of propranolol. Propranolol has been in use since the late 1960s. INDERAL® LA was approved later and has been used to treat high blood pressure for over 25 years. INDERAL® LA offers a once-daily treatment option that releases propranolol at a controlled rate.

In addition to high blood pressure, INDERAL® LA is approved to treat angina pectoris, prevent migraine, and for a heart condition called hypertrophic subaortic stenosis. INDERAL® LA is not indicated in the management of hypertensive emergencies.

INDERAL® LA is available in 60, 80, 120, and 160 mg long-acting capsules.

Blood pressure is determined by the amount of blood your heart pumps and the amount of resistance to blood flow in your arteries. The more blood your heart pumps and the narrower your arteries, the higher your blood pressure. Uncontrolled high blood pressure can increase your risk for serious health problems like heart attack and stroke.

High blood pressure is a common condition. Millions of Americans suffer from it. Generally, high blood pressure develops over many years. Since there are usually no symptoms, you may not know that you have high blood pressure until you are diagnosed. Fortunately, there are treatment options.

Angina pectoris is the medical term for chest pain or discomfort usually due to coronary artery disease. It occurs when the heart muscle doesn’t get as much blood, and therefore as much oxygen, as it needs. This usually happens because one or more of the heart’s arteries has narrowed or is blocked. During a typical angina attack, a person feels uncomfortable pressure, fullness, squeezing, or pain in the center of the chest. This discomfort may also be felt in the neck, jaw, shoulder, back, or arm.

Angina is a signal that the heart needs more blood. Exercise, strong emotions, or extreme temperatures can trigger an angina attack. Some people, such as those who experience coronary artery spasm, may have an angina attack even when they’re resting.

High blood pressure can have severe health consequences. The higher your blood pressure and the longer it goes uncontrolled, the more severe these consequences can be. High blood pressure is a risk factor for the following conditions, as well as others:

  • Heart attack
  • Stroke

As with all prescription medications, you should follow your healthcare provider’s guidance when taking INDERAL® LA. Here are some important tips for taking INDERAL® LA:

  • Take one capsule of INDERAL® LA daily. Do not take more than one capsule daily without your healthcare provider’s knowledge and direction.
  • Do not stop taking INDERAL® LA without consulting with your healthcare provider first. Some patients who suddenly stop taking INDERAL® LA may be at increased risk of chest pain or heart attack.
  • Let your healthcare provider and pharmacist know about all of the other medications you are taking (prescription and non-prescription drugs) before starting INDERAL® LA.
  • Make sure your healthcare provider knows your medical history. INDERAL® LA is not recommended for patients who have been previously diagnosed with a certain type of arrhythmia or with bronchial asthma or COPD.
  • Store your INDERAL® LA in a cool, dry place where it will be secure.

INDICATIONS AND USAGE: INDERAL® LA (propranolol hydrochloride) Long-Acting Capsules are a beta adrenergic blocker indicated for the treatment of high blood pressure (hypertension), angina pectoris and hypertrophic subaortic stenosis. INDERAL® LA is also indicated for the prevention of migraine headache. Lowering blood pressure can reduce the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions.

LIMITATION OF USE: INDERAL® LA is not indicated in the management of hypertensive emergencies.

Important Safety Information About INDERAL® LA

What is INDERAL® LA?

INDERAL® LA is a prescription medicine that belongs to a group of medicines called “beta blockers” used to treat high blood pressure. INDERAL® LA lowers blood pressure, which can reduce the risk of strokes and heart attack. INDERAL® LA is not to be used to treat high blood pressure emergencies.

INDERAL® LA can decrease frequency of chest pain and increase ability to exercise in patients with heart disease.

INDERAL® LA can prevent common migraine headache; however, the ability of INDERAL® LA to treat an existing migraine is not known.

INDERAL® LA improves heart function in patients with hypertrophic subaortic stenosis (obstruction of blood flow from the heart).

What is the most important information I should know about INDERAL® LA?

WARNING: Do not stop taking INDERAL® LA suddenly, as heart attacks and worsening of chest pain have occurred. If your doctor decides you should stop taking INDERAL® LA, you may be instructed to slowly reduce your dose over a period of time before stopping it completely.

Do not use INDERAL® LA if you have:

  • Abnormal heart rhythms caused by a malfunction of the heart’s primary pacemaker
  • Sudden and severe drop in blood pressure and bloodflow through the body because the heart is not pumping normally
  • Uncontrolled heart failure
  • Asthma
  • Any allergies to INDERAL® LA ingredients. The active ingredient is propranolol.

What should I tell my healthcare provider before taking INDERAL® LA?

Before starting INDERAL® LA, tell your healthcare provider about all of your medical conditions, including if you:

  • Have heart failure
  • Are scheduled for surgery and will be given anesthetic agents
  • Have diabetes and take medicine to control blood sugar
  • Have thyroid problems
  • Have irregular heartbeats
  • Have liver or kidney problems
  • Have had allergic reactions to medications or have allergies
  • Have asthma or other lung problems (such as bronchitis or emphysema)
  • Are pregnant or trying to become pregnant. It is not known if INDERAL® LA is safe for your unborn baby. Talk with your doctor about the best way to treat your high blood pressure during pregnancy.
  • Are breastfeeding or plan to breastfeed. Propranolol passes into your breastmilk. Talk with your doctor about the best way to feed your baby during treatment with INDERAL® LA.

Tell your healthcare provider about all of the prescription or over-the-counter medications, vitamins, or herbal supplements you are taking including:

  • Wafarin
  • Propafenone
  • Other beta blockers and alpha blockers such as: clonidine (Catapres, Kapvay, Nexiclon), prazosin (Minipress, Vasoflex, Lentopres and Hypovase)
  • The heart failure drug dobutamine
  • Antidepressants
  • Non-steroidal anti-inflammatory drugs (NSAIDS) for pain relief, fever reduction, and reducing inflammation including over-the-counter ibuprofen (e.g. Advil, Motrin)
  • Migraine drugs such as zolmitropan or rizatriptan

This list is not complete and not all possible interactions are listed. Other drugs may interact with INDERAL® LA, including prescription and over-the-counter medicines, vitamins, and herbal products.

What are the possible side effects of INDERAL® LA?

The most common side effects people taking INDERAL® LA report are tiredness, dizziness (if you feel dizzy, sit or lie down and tell your doctor right away), and constipation.

Other possible side effects include difficulty breathing, tightness in the chest, heart failure, severe or prolonged low blood sugar, overactive thyroid, especially a fast heartbeat, and certain abnormal heart rhythms.

What other information do I need to know about taking INDERAL® LA?

  • INDERAL® LA should be taken orally.
  • Do not stop taking INDERAL® LA suddenly. You could have chest pain or a heart attack. If your doctor decides that you should stop taking INDERAL® LA, he or she will lower your dose slowly and over time.
  • Your doctor will tell you how much INDERAL® LA to take and how often. Your doctor may start with a low dose and raise it over time.
  • Do not stop taking INDERAL® LA or change your dose without talking with your doctor.
  • Do not drive or use machinery until you are sure you can perform activities safely.
  • If you have heart failure, consult your physician if you experience weight gain or increasing shortness of breath.
  • If you have diabetes, consult your physician if you experience changes in your blood sugar levels. Your risk for getting severe or prolonged low blood sugar increases while taking INDERAL® LA. If severe low blood sugar occurs, seek emergency treatment.
  • People taking INDERAL® LA may be less responsive to epinephrine used to treat a severe anaphylactic reaction.
  • If you take too much INDERAL® LA call your doctor or poison control center right away.
